*URGENT Opening* for a *Pediatric Case Manager Registered Nurse* (*ONSITE)* in a Baltimore Hospital!!!! Interested candidates should have recent Case management/discharge planning experience. ICU and/or Pediatric experience referred.

Requirements of the Pediatric Case Manager:
* *MD or compact Registered Nurse*
* Discharge planning experience in Hospital setting required
* Pediatric and/or ICU experience preferred
* EPIC experience preferred

*Pay & Perks of Registered Nurse Homecare:*
* *Registered Nurse Pay: $43 per hour*
* Good work/life balance: Only 40 hours per week.
* 12 week assignments give you the flexibility to see if the company is a good fit.
* Possible temporary to permanent.
* *Shifts for Registered Nurse Pediatric Case Manager:* M-F, 8a-4:30p

*RN Case Manager Position Objective:*
* The Pediatric Case Manager actively coordinates the needs of patients who meet medical admission criteria. Responsibilities include patient assessments, identification of discharge goals, case management planning, and implementation of interventions, monitoring/tracking care progression, and ensuring follow-up and coordination of services.
