Nationwide Therapy Group is seeking a travel nurse RN Case Management for a travel nursing job in Greensboro, North Carolina.

• Employment Type: Travel

• Assess patient needs and collaborate with physicians, nursing staff, social workers, and outside providers to formulate comprehensive discharge plans.
• Order and coordinate Durable Medical Equipment (DME) , home health, SNF/rehab placement, or hospice as needed.
• Communicate and advocate for patients and families in navigating post-acute care transitions.
• Apply CMS guidelines and payer-specific criteria to determine medical necessity.
• Participate in regulatory documentation and compliance with Medicare, Medicaid, and private payers.
• Maintain updated and accurate records using EPIC .
• Perform needs assessments , review clinical documentation, and ensure authorization requests align with insurance guidelines.
• Assist in appeals and denials management if needed.
• Serve as a resource and liaison for weekend staff regarding discharge planning and payer compliance.
• Leverage previous charge nurse experience to independently manage unit-level needs with minimal supervision.
• Strong knowledge of:
• CMS/Medicare/Medicaid regulations
• DRG, ICD-10, CPT coding
• HEDIS, NCQA, HIPAA, OSHA, and Joint Commission standards

Must pass Cone Health EKG exam\n\nEPIC proficiency required
