Current CPR certification preferred. Must-Have: Meets state-specific requirements, such as LCSW Job Description: - Demonstrate familiarity with dialysis procedures and the necessary skills and judgment to care for dialysis patients.- Ensure patient rights and privacy, creating a safe, secure environment that supports the dignity and well-being of all patients.- Cooperate with co-workers to plan and complete job duties with supervisory direction and appropriate judgment.- Comply with all company policies and procedures, as well as federal, state, OSHA, and other agency regulations.- Handle patient information confidentially and maintain HIPAA compliance.- Communicate with tact and professionalism to promote positive relations with patients, their families, physicians, co-workers, and service providers.- Provide comprehensive psychosocial assessments on each patient upon admission and annually.- Assist patients and family members in adjusting to dialysis and understanding their rights, facility rules, grievance procedures, and policies.- Help patients with psychosocial issues related to dialysis throughout their treatment.- Support patients in obtaining financial assistance, medical insurance, transportation, travel, and home healthcare.- Develop or improve facility and community resources to meet patient needs.- Participate as a member of the Interdisciplinary Team in patient assessments and care planning.- Maintain records in accordance with facility policy and ESRD regulations, completing progress notes at least quarterly.- Contribute to the facility's Continuous Quality Improvement process as a member of the Total Quality Management team.- Assist with patient education and in-service programs.- Stay informed about research and developments in nephrology social work, attending relevant meetings and conferences.- Work with federal, state, and local agencies to improve and secure resources for patients.- Ensure quality assurance and compliance with all regulatory requirements.- Address or escalate issues regarding complaints, problems, and concerns as appropriate.
