What You'll Do: 

• Travel daily to assigned MN service areas to visit clients in their private homes
• Conduct functional and cognitive evaluations to develop tailored therapy plans aligned with Medicaid and state guidelines
• Administer restorative and maintenance treatments; supervise OTA contributions and ensure compliance with billing regulations
• Provide guidance on home modifications, assistive devices, and caregiver training to enhance functional outcomes
• Maintain accurate records of services and collaborate with care teams to align therapy with broader treatment goals

What You'll Need: 

• Active MN Occupational Therapy License
• Graduate of an accredited OT program
• CPR certification
• Reliable vehicle with valid license and insurance
• Home care or geriatric experience is preferred
