Requirements:Skills and Qualifications:
- High school diploma or equivalent; relevant certification or degree in healthcare or social services preferred.
- CHW Certification is Mandatory and Proof of certification required
- Strong communication and interpersonal skills.
- Ability to work effectively with diverse populations.
- Knowledge of community resources and healthcare services.
- Experience in health education or community outreach is a plus.
- Strong organizational and time management skills.
- Compassionate and empathetic approach to patient care.
