About the Role:

The Health Coach regularly interacts with practice members and is responsible for ensuring all practice members feel important, appreciated, welcomed, remembered, and cared for. She/he is responsible for providing an unmatched client experience, building lasting connections, and helping all of our practice members achieve the highest possible success in their wellness programs. She/he is also a steward for bringing in referrals to the practice to continue organically growing Carolinas Natural Health Center’s business.

Key Responsibilities:

• Onboarding new members into wellness programs
• Reads all progress reports and reaches out to members who have questions, concerns, or who may be struggling
• Meets with practice members regularly through in-person, virtual, and phone check-ins.
• Establishes personal rapport with members in a health coaching capacity, helping them stay on track with the program and requirements laid out by the practitioners.
• Helps practice members explore their strengths and encourages them to stay on track with their health plans, even when it’s challenging
• Empower practice members by helping to enforce their health goals by coaching the practice members and their families to understand their health challenges.
• Helps practice members develop self-care and self-regulation skills.
• Establishes a compassionate environment by providing emotional, psychological, and spiritual support to practice members and their families.
• Assures quality of care by measuring health outcomes against practice member care goals and standards; collaborating with the practitioners in any instance where adjustments need to be made
• Refers to other health care providers as indicated
• Documents practice member interactions and care in practice EMR
• Maintains practice member confidence and protects operations by keeping information confidential
• Continued education and professional growth
• Contributes to the team culture of excellence in member care
• Contributes to the practice of social media and marketing activities
• Contributes to a culture of ongoing process improvement and patient experience improvement
• Any other duties as assigned

• Comfortable holding people accountable without judgment or criticism
• Excel at asking direct questions to get to the heart of the matter
• Ability to track and measure practice member progress and results
• Have a broad understanding of specific health and wellness subjects designed to help practice members navigate their respective journeys
• Computer efficiency - Google Workspace, mastering office EHR systems, and using project management software like Asana
