QUALIFICATIONS
LVN with Knowledge of healthcare reimbursement, case management and discharge planning is required.

Team Touchstone Directors of Care Solutions/Case Managers are responsible for the following:
Exercise clinical expertise, knowledge and decision making to ensure successful discharge planning and care management
Ensures necessary information is communicated to IDT, direct care staff, patient and responsible party/caregiver
Facilitates the 72-hour and Care Connect meetings
Responsible for the ongoing communication to the case management team
Coordinates Care reviews
Guarantees patients are monitored for change of condition
Collaborate with the interdisciplinary team to develop individualized plans of care
Leads successful transition of care though continued recovery once at home
Provides education and support to patients and families about care processes, discharge expectations and resources
Serve as member of the QAPI committee
Director of Care Solutions is aware of the Balanced Scorecard and how their position contributes to success in Balanced Scorecard measures
